Another failure of opinion polls in the US

The Michigan Republican primary was another instance that opinion polls seem to have failed in the US.
The result of the actual voting was
Romney: 39%, McCain: 30%, Huckabee: 16%, Paul: 6%, Thompson: 4%, Giuliani: 3%
and the results of the last polls were
